Summary

The Supervisory Social Worker is assigned to Post 9/11 Military 2 VA Case Management Program within the North Florida/South Georgia Veterans Health System (NF/SGVHS) and is responsible for the system wide implementation of the program. The Supervisory Social Worker manages supervisory, administrative and clinical aspects of the interdisciplinary staff and programs. This position is also known and serves as the facility Post 9/11 Military 2 VA Case Management Program Manager.

The Supervisory Social Worker is the supervisor for the Post 9/11 Military 2 VA Case Management team which assists with the transition to VAMC health care from Military Treatment Facilities (MTF) and outpatient Service Members who present to the VAMC for care. This program provides care for those who served on or after September 11, 2001 including those who served in support of Operations Enduring Freedom, Iraqi Freedom and New Dawn and are ill or injured and transitioning to VA.

Many transitioning Service Members and Veterans suffer from multiple complex health and mental health problems, including but not limited to, Traumatic Brain Injury (TBI), Spinal Cord Injury (SCI), amputations, burns, loss of vision, combat stress and Post-Traumatic Stress Disorder (PTSD). This team ensures the care of all transitioning service members and Veterans is well-coordinated and that those who have a need for care or case management receive those services from a VA case manager.

Duties include but are not limited to:

  • Plans, develops, implements, and evaluates the facility Post 9/11 Military 2 VA Case Management Program to ensure continual quality improvement. Oversees all transition activities and the coordination of care and services for Post 9/11 Service Members and Veterans.
  • Leads the Case Management team, which includes the clinical case managers and Transition Patient Advocate; ensuring policies and procedures are in place and facilitating communication among the team.
  • Manages the daily operations and work assignments of the clinical staff (social workers and nurses) and their impact on the daily operations. Develops policies and procedures for program operation. Prepares reports and statistics for service line, facility, VISN, and national use. Utilizes funding data related to the programs to improve daily operations.
  • Facilitates efficient and appropriate delivery of services across episodes of care within the Medical Center and Community-based Outpatient Clinics (CBOC), including Mental Health Services.
  • Responsible for the overall oversight of clinical services provided by staff, must assess on-going training needs, and completes competencies/ performance reviews/plans
  • Provides staff training and development, along with skill in interpersonal relationships in dealing with employees, team leaders and managers.
  • Performs coordination responsibilities, in consultation with other leadership, that includes program resources in the community, liaison with community resources, staff education regarding Post 9/11 Military 2 VA Case Management Programs and Services both within and outside of VA, referral processes for VA/Community programs, and communication of standards, policies and procedures.
  • Serves as the Post 9/11 Military 2 VA Case Management liaison to other Services or care-lines, other facilities, Veterans Integrated Service Network (VISN) offices, VHA Central Office and VA Liaisons for Healthcare at the Military Treatment Facilities (MTFs).
  • Develops a business plan that outlines the program's missions, strategic goals and objectives, forecasts the yearly budget and describes all appropriate resources for achieving stated outcomes.
  • Collaborates with other services regarding policy development and with Fiscal Services regarding funding expenditures
  • Develops and maintaining effective partnerships throughout the facility and satellite clinics including Primary Care, Veteran Readjustment Centers, Specialty Care Clinics, Facility Patient Advocates, VBA and the Business Office
  • Ensures all program metrics are established and met
  • Clinical assessment and treatment services are provided

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f).
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
    • May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.

GS-12 Supervisory Social Worker candidates must have the following:
  • Experience/Education: Applicants applying at the GS-12 grade level must have 1 year of experience, equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Training or additional certs must be submitted with application.
AND
  • Licensure/Certification: At this grade level, social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Advanced License must be submitted with application.

Preferred experience:
  • Previous supervisory experience.
  • Prefer experience with program development and process improvement.
  • Prefer extensive post licensure experience providing supervision, knowledge of latest version of DSM, knowledge of transition from military to civilian life, experience with medical and/or case management, providing crisis and chronic behavioral health interventions, experience with providing training, orientation, and strong clinical and administrative skills.

Work is typically performed in a clinical setting that exposes the incumbent to the communicable diseases and behavioral problems of the client population. Able to meet the physical demands of the position, to include sitting, standing, bending, frequent walking per shift, and lifting up to 25 pounds.
