The VA Hudson Valley HCS is currently recruiting for one (1) Full-Time LPN for the Women Veteran Health Program. The VA Hudson Valley Health Care System offers a full range of acute and chronic care services for Veterans at various stages of life residing in six adjacent counties, including Dutchess, Westchester, Putnam, Orange, Rockland and Sullivan serving over 400,000 Veterans. Services include, but are not limited to, Medicine, Primary Care, Specialty Care, Urgent Care, Ambulatory Procedures, Sterile Processing, Psychiatry, Rehabilitative Medicine, Long Term Care, Women's Health, Blind Rehabilitation, Mental Health, Substance Abuse, PTSD, Domiciliary, Laboratory and Home-Based Primary Care.
The LPN Women Veteran Health position is aligned under the Chief of Staff Service and provides patient centered care to multiple outpatient Clinics using the Patient Aligned Care Team model. The Primary Care clinics are located at the two (2) main campuses and seven (7) Community Based Outpatient Clinics (CBOC). The LPN is expected to have broad working knowledge of nursing procedures and perform all duties within the scope of their license; however, completed work may require routine or detailed higher-level review depending on the complexity of the duties involved.
The Licensed Practical Nurse (LPN) working in the GYN clinics is responsible for providing safe, effective clinical and administrative support in a wide variety of patient care and related activities. The LPN functions as a member of the health care team by demonstrating the practical nursing knowledge and skills necessary in carrying out complex practical nursing care assignments within the GYN clinics.
The purpose of this position is to be a productive team member carrying out and ensuring quality and timely delivery of Veteran centric care. The incumbent must possess the personal qualities, desire and attitude necessary to adapt to rapidly changing demands on time and workload and have necessary skills to perform specific tasks/tests/procedures to meet the Veteran's complex, healthcare needs.
Under the direction of the Registered Professional nurse, the LPN provides a broad range of direct, age specific patient care activities and supportive services, as identified in age-specific competencies, including demonstrating knowledge of changes associated with aging and principles of growth and development relevant to the adult and geriatric age groups. Using traditional face to face and/or telehealth modalities, demonstrates the ability to interpret, report and document data about the patient's status; the ability to identify age-specific needs and provide appropriate care based upon the age-related factors noted including assisting with procedures, providing patient education and other duties as needed.
Work Schedule: Monday, Tuesday, Wednesday 8am to 4:30pm
