A. The senior social worker for the Home Based Primary Care (HBPC) program for designated locations) oversees all program activities and the coordination of care and services for HBPC Veterans treated at specific locations designated by the HBPC Program Director, Fayetteville VA Medical Center. The HBPC program is a program under the department of Geriatrics and Extended Care. HBPC Veterans health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving. The Veterans are clustered in the immediate area of the clinic. The incumbent facilitates efficient and appropriate delivery of longitudinal services. The incumbent plans, develops, implements, and evaluates the HBPC Program to ensure continual quality improvement and excellence in customer service. The incumbent serves as the team leader of the interdisciplinary treatment teams, coordinates the screening of referrals to the HBPC Program and participates in other planning meetings as necessary. The work directly affects the health and well-being of the patients served and relationships with community organizations and stakeholders.
B. The senior social worker has programmatic responsibility for the HBPC Program and is assigned to designated locations as per the HBPC Program Director. The incumbent is a master's prepared social worker, practicing at the advanced level and has specialized knowledge of practice related to outpatient care of Veterans in their homes.
C. Work involves oversight of a complex array of services, both clinical and administrative, for HBPC Veterans treated by the Fayetteville VA Medical Center's coastal clinics. Work requires the exercise of mature professional judgment and flexible use of administrative, leadership, and social work skills. Decisions concerning planning, organizing, and implementing the plans are often complicated by the various agencies and individuals involved in the care plan.
The senior social worker is responsible for providing for the medical / psychosocial needs of Veteran enrolled in the program.
