Vacancy located within the Tennessee Valley Healthcare System at the Clarksville Community Based Outpatient Clinic, located in Clarksville, Tennessee. The Clarksville Clinic provides primary care, along with specialty care services and operates in a multidisciplinary PACT model. Education Debt Reduction Program (EDRP) is Authorized. This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. To participate in the program, you must meet specific individual eligibility requirements in accordance with VHA Policy 1021 and submit your application within four months of appointment. The final award amount (up to $200,000) and eligibility period (one to five years) is determined by the VHA Education Loan Repayment Services office. Candidates must have an internal medicine or family practice background, with greater than two years of experience preferred. We are seeking a candidate with a passion for serving Veterans and a desire to work within a newly built, state of the art outpatient clinic that incorporates traditional medicine, holistic medicine and telehealth medicine.
Duties include, but are not limited to:
Provide total care of Veteran's medical needs by performing history and physical exams , ordering appropriate lab tests, radiology studies, screening studies. This includes evaluating those tests, developing a treatment and follow-up plan to include medication, medical equipment, and home based interventions within the guidelines laid down by the supervising physician. This care will also include the outpatient follow up visits, post discharge follow-up and managing care with specialty and sub-specialty medical providers. This patient care will be provided through face to face visits and other modalities such a telephone clinics, VA Video Connect clinics, group clinics, secure messaging, etc.- Evaluate and treat acute and chronic health problems for scheduled and unscheduled patients in the clinic setting. Ensures that each patient is provided a list of current medications and an appointment list at close-out.- Ensures that patient encounters, notes, orders, and consults are appropriately closed out including disposition that clearly indicates next steps of care. Also responsible for ensuring coding is correct and in accordance with coding guidelines.- Performs office procedures contained within their privileges.- Communicates test results to patients.- Assist in preparation of the patient and/or his/her family to include, but not limited to, teaching (to include health promotion and disease prevention activities), accumulating data and medical problems, and presenting patient centered discussion and whole health of medical problems for appraisal and scheduling patients within appropriate Primary Care departmental services.- Uses data sets contained within the VHA almanac, Compass, Electronic Quality Measures and High Risk registries to identify and more highly manage patients at high risk and/or that have recurring outcome values that outside the norm.- Maintains skills, proficiencies and professional knowledge required for licensing and clinical scope of practice.- Provides support to APRN's working within clinics.- Required to attend staff meetings, may serve on committees, teams, work-groups and complete peer-reviews as requested. The Provider will complete CME, VA Training, and other mandatory trainings as assigned in TMS.- Provider will float between the Clarksville VA clinic and Dover VA clinic. Travel is associated with this position.
Occasional travel to Dover, TN will be necessary to help coverage when other providers call out sick or are on leave.
Work Schedule: Monday-Friday (8:00 a.m.to 4:30 p.m.)
