VA New Jersey HCS Radiation Oncology Department is a training site for resident Physicians
from the Radiation Oncology Program at Rutgers Robert Wood Johnson Medical School. To
meet our clinical and educational obligations, we require the services of qualified Physician-
(Radiation Oncologist), to support the Radiation Oncology service at the East Orange Campus
of VA NJHCS. The Radiation Oncology Team is made up of highly skilled, caring professionals
including radiation oncologists, nurse practitioners, nurses, health techs, medical physicists,
radiation therapists, dosimetrists, biologists, administrators, social workers, dieticians, nurse
navigators and oncology psychologists.
Scope of service includes but is not limited to:
Clinical Evaluation
The Physician-(Radiation Oncologist) initial evaluation of the patient includes history,
physical examination, review of pertinent diagnostic studies and reports, and communication
with the referring physician and other appropriate physicians involved in the patient's care.
Establishing Treatment Goals
The Physician- (Radiation Oncologist) will define treatment goals (curative, palliative,
adjuvant, pre-op or post op, or to establish local tumor control) as clearly as possible .He/she
will determine need for additional procedures such as placement of fiducial markers or rectal
spacers. He/she will also determine the need for addition of chemotherapy, immunotherapy
or hormonal therapy or a combination of same and the timing of such in conjunction with the
medical oncologist as needed.
Simulation of Treatment
Simulation is the process of establishing and documenting the appropriate volume to be
treated and identifying the normal structures within or adjacent to this volume. During
simulation, The Physician- (Radiation Oncologist) will determine the optimal patient
positioning, and immobilization devices required. The Radiation therapy modality to be used
is also determined since it relates to the scan. EBRT: 3D, IMRT, SBRT, or SRS or electron beam
therapy. Brachytherapy:LDR or HDR.
Treatment Planning
The cognitive process of treatment planning requires the Physician- (Radiation Oncologist) to
have knowledge of the natural history of the tumor to be treated and to determine the tumor
site, its extent, and its relationship with adjacent normal tissues and tissue tolerance. He/she
may use fusion of MR or PET with the planning scan to contour. He/she contours the normal
tissues, organs at risk and target volumes with the dosimetrist. The plan is developed based
on dose/ volume objectives and constraints for the specific cancer type and stage. Gantry and
collimator angles and couch positions are determined at this time as well as need for internal
or external compensation.
Informed Consent
Prior to simulation and treatment, informed consent must be obtained and documented by
the Physician- (Radiation Oncologist), or his/her designee consistent with departmental
standard operating procedure.
Monitoring of Treatment Delivery
Treatment management is an ongoing dynamic process in which the Physician- (Radiation
Oncologist) oversees treatment delivery as well as any potential symptoms or complication
resulting from the patient's disease or from treatment.
Follow-up of Patients after Completion of Treatment
Upon completion of treatment a summary is created. The patient then receives a follow up
appt. consistent with department policy and NCCN guidelines.
Participation in Clinical Trials
The Physician- (Radiation Oncologist) is expected to offer enrollment in clinical trials to all
eligible patients and to participate as a PI or Co-PI on department initiated trials.
Timeliness Requirements
The Physician- (Radiation Oncologist) will follow the requirements outlined in department
standard operating procedures which incorporate standard VA requirements as well as those
standard in the specialty of radiation oncology.
Documentation Requirements
The Physician- (Radiation Oncologist) must apply for access to the VA Intranet system and
demonstrate competence in accessing healthcare system policies and other key documents
archived on the Intranet. Additionally, the Physician- (Radiation Oncologist) must read and
respond to VA e-mail on a daily basis.
