Incumbent provides psychosocial services to the veteran population served at the Transitional Care Program, the Menlo Park Division of the VA Palo Alto Health Care System. Services include provision of high quality case management, clinical treatment, advocacy, outreach and coordination of linkages to other appropriate VA or community service providers, agencies and placement services as needed by the client. This is accomplished in collaboration with other members of the interdisciplinary treatment team and community agencies and providers. The incumbent evaluates and provides complex, multiple services to individuals and families with a variety of social, emotional and health related problems. Incumbent may provide short term follow up case management for Veterans who are immediately discharged from the Transitional Care Program and require specialized or complex clinical services in order to ensure a safe discharge. Services are timely, comprehensive, and effectively coordinated with services provided by other health professionals.
Duties include but are limited to the following:
- Incumbent serves as program coordinator of the Transitional Care Program and is administratively responsible for a clinical program providing treatment to Veterans in the Community Living Center.
- Incumbent manages the daily operations of the program, and provides direct client care services in the program area.
- Incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use.
- They may be responsible for the program's budget and various control points assigned to the program within the assigned areas.
