The Licensed Practical Nurse (LPN) is responsible and accountable for providing nursing care to individual and groups of patients for an aging, adult population. The incumbent must demonstrate good clinical practice, problem-solving, and decision-making skills. The LPN appropriately seeks assistance and guidance by informing the RN or the medical provider of changes in the patient's condition requiring a higher level of intervention. He/ she is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of a core team consisting of the Patient, Primary Care Provider, RN Care Manager, and Medical Support Assistant. The incumbent is also a part of the expanded team consisting of family/ caregiver, internal and community based services involved in providing care to the patient. The PACT support team will assist with delivery of comprehensive primary care and gender specific specialty care through traditional modalities of care as well as via Tele-health and shared medical appointments. The team will also assist the clinic and Women's Health Program with care coordination, performance/quality initiatives, implementation of best practices and pilot projects, and training of primary care staff, medical residents and nursing students. The incumbent will complete other duties as assigned. The incumbent may be assigned and required to travel and work at various CBOCs as needed and work extended clinic hours as needed. The LPN position is clinically and administratively responsible to the Nurse Manager of Primary Care.
Work Schedule: Monday through Friday, 08:00 A.M. to 4:30 P.M., schedule may vary based needs of Primary Care Service line.

The reason for temporary assignment is based on COVID-19 needs. The length of appointment is not to exceed one (1) year, may be extended until 9/30/2021 or extended for an additional year. May be terminated or extended at any time based on the needs of the facility.
