The TAVR Coordinator practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP provides advanced clinical practice, consultation, and management.
The TAVR Coordinator is granted full practice authority according to the VA medical regulations, and a graduate of a school of professional nursing approved by State accrediting agency at the time the program was completed by the applicant and has met licensure requirements for practice in accordance with VA Handbook 5005, Nurse Qualification Standards dated March 17, 2009, and the area of assignment.
Degrees as specified shall be from colleges or universities with degree programs for graduate nurses fully accredited by the National League for Nursing Accrediting Commission (NLNAC), or the Commission on Collegiate Nursing Education (CCNE) at the time the degree was granted.
The TAVR Coordinator should be a citizen of the United States (non-citizens may be appointed when no qualified U.S. citizens are available in accordance with regulations issued by the Under Secretary for Health or designee). Physical standards in accordance VA Directive and Handbook 5019 must be met. The minimal educational requirement is a Master's Degree in Nursing. Practice requirements include BLS and/or ACLS where appropriate.
The TAVR Coordinator is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a specific population of patients.
Major duties and responsibilities of the position:
- Clinical team lead for development of TAVR Program and works collaboratively with the team.
- Coordination and presentation of education for TAVR program.
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Provides assessments, evaluations, and care coordination of Veterans.
Initial screening including reviewing all pertinent clinical records and images to determine candidacy.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Review Aortic Valve Coordinator Consults placed by other providers at Hines VA and within VISN 12.
-Establish components of care including clinic, testing, procedure environment, and inpatient care.
- Maintenance of patients in all phases of the screening process.
- Primary point of contact for patients and TAVR device vendors.
- Coordination, hosting, and documentation of multidisciplinary heart team.
- Schedules with OR and ensures necessary equipment is acquired and available for procedures.
- Orders and interprets labs, pre-operative testing, and diagnostic studies.
- Coordinate Care in the Community with Community Care Office for all TAVR referrals, including reviewof referrals placed by another provider.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Aides in health promotion, disease prevention, and health education.
- Initiates referrals/consults for services, serving as a VA and community liaison.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
- Provides consultation regarding Veterans with surgical specialty problems.
- Meet standards and regulatory requirements, such as Joint Commission, Performance Measures, and Clinical Practice Guidelines.
- Collaborates with the interdisciplinary team in planning, implementation, and evaluation of surgical care for Veterans.
- Assist with all necessary equipment acquisition from CPRC through contracting.
- Discharge Planning including education and follow-up care.
