Duties
Patient Safety Manager (PSM) Registered Nurse (RN) executes position responsibilities that demonstrate beginning leadership skills and participates in specific program experiences. They demonstrate creative approaches to the management of program aspects of complex patient care. The PSM demonstrates performance and beginning leadership skills that start to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care. The PSM functions as the lead manager and implementer of patient safety policies and activities developed by the National Center for Patient Safety (NCPS) and is the facility's key person to help build and implement patient safety systems and promote the culture of safety within Veterans Affairs (VA) facilities and for VA patients in all healthcare settings. Direct support is provided to the development, maintenance, and operation of the VHA's patient safety process at the facility level. This includes working with all levels of staff within the facility, including top-level management, Root Cause Analysis (RCA) team members, RCA team leaders, and RCA recorders. They are responsible for a facility wide program for all services within the organization that meets the requirements of National Directives. The PSM will oversee the provision of staff education, orientation, competencies and providing quality improvement consultation related to the program specifically. The PSM continuously evaluates the program, collecting and submitting facility performance measures.
The PSM is responsible for ensuring timely investigation of patient safety reports entered into Joint Patient Safety Reporting (JPSR) at their facility. PSM are expected to perform a proactive risk analysis, healthcare failure mode effects analysis (HFEMA) for the purpose of improving care to patient groups maximizing utilization of resources and enhancing the Patient Safety program. They are responsible for maintaining clear communication between the facility and the VISN, to communicate relevant patient safety information and promote organizational learning.

Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased medical facilities. HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities. HPTs may be paid or unpaid and include residents, interns, fellows and students. HCP also includes VHA personnel providing home-based care to Veterans and drivers and other personnel whose duties put them in contact with patients outside VA medical facilities.
