The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position.
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
Major duties and responsibilities of the position:
This position will be the primary provider for our Polytrauma program including ischemic brain injury management and will be the provider in our amputee clinic working with our CPO, prosthetic department and PMR staff to manage and address Veterans with prosthetic needs. Willingness to train in both programs and ability to manage a diverse case load is a must.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with the interdisciplinary team.
- Orders and interprets labs, tests, and diagnostic studies.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
- In charge of Clinical Nurse Management for Polytrauma.
- Independently provides direct patient care and coordinates the interdisciplinary care needs team meetings for the polytrauma, rehabilitation medicine and amputee.
- Prescribes, orders, recommends durable medical and adaptive equipment for purchase that aids in diagnosis, treatment, education and prevention.
- Provides leadership for outpatient amputee services and provides direct and comprehensive patient care to the veterans are involved in the clinics.
- Diagnose, treat, and monitor a wide range of medically complex conditions with varied stages of chronic diseases such as musculoskeletal, neurological, wound/edema, polytrauma, and brain injuries.
- Oversight of all amputation-related care needs for Veterans seen in the Outpatient Amputation Specialty Clinic.
- Completing the prosthetic prescription and all required documentation and coding associated with Outpatient Amputation Specialty Clinic visit. .Assuring medical appropriateness and readiness for prosthetic restoration.
- Addressing amputation-specific medical conditions.
- Referring the Veteran for additional services as indicated.
