The HUD/VASH Outreach RN acts as a healthcare consultant for Veterans needing clinical/nursing services while living in the community (i.e., diabetes care, medication instruction and appropriate administration, monitoring substance use disorder (SUD) compliance and blood pressure monitoring).
The HUD/VASH Outreach RN provides clinical services to client such as guidance, emotional support, referrals to agency organizations, community resources and other assistance.
The HUD/VASH Outreach RN in collaboration with the HUD/VASH team performs local outreach for homeless Veterans at various times of the year. These local outreach events provide the community with information regarding the goal of the HUD/VASH program as well as seek resources for Veterans to receive supportive housing.
The HUD/VASH Outreach RN is responsible for the completion of the electronic medical record (EMR) and evaluation and monitoring tools to ensure the delivery of care and outcomes.
The HUD/VASH RN will operate a government vehicle to carry out job responsibilities, including the driving and management of the Mobile Medical Unit.
