Duties Include, but are not limited to:
- Responds to all incoming referrals for the various Homeless Program services- Gathering information regarding the Veteran's clinical background and referral needs and facilitates assessment times and/or additional referrals to appropriate services within the facility or to external partners.
- Facilitates utilization of resources by assessing eligibility and making referrals to a variety of VA resources and community agencies for assistance with social, emotional, financial, and physical health problems.
- Provides assistance, as needed, with applications for benefits, services, and resources, as well as negotiating service systems.
- Coordinates interest lists for all Homeless Program services, routinely reviewing them for program prioritization according to individual program requirements.
- Provide complex crisis intervention and stabilization to Veterans who are in psychological distress, as needed.
- Serves as an active member of the Homeless Program Interdisciplinary team to provide information on eligibility status and the status of referrals to VA resources and community agencies to assist the team in updating treatment plans and providing clinical care.
- Provides subject matter consultation to colleagues and students related to eligibility for homeless program services, community resources, and other homeless Veteran specific topics.
- Records clinical information in the Veteran's electronic medical record in accordance with prescribed standards.
- Collects and maintains required data related to referrals, enrollments, and ineligible Veterans.
- Participates on a variety of service and facility committees, task groups, or special projects, as assigned by the supervisor, Homeless Program Manager, or Service Chief.
- Participates in community or field activities in a variety of situations to better coordinate care and resources for Veterans.
- Provides consultation to community partners on Homeless Program services, eligibility, and other homeless Veteran specific topics.
- Attends appropriate educational programs, seminars, and trainings
- Uses word processing software to execute several office automation functions such as storing and retrieving electronic documents and files, activating printers, inserting and deleting text, formatting letters, reports, and memoranda, and transmitting and receiving e-mail and faxes.
- Uses various databases to access information related to Veteran service history to determine eligibility for healthcare and various Homeless Program services.
- Collaborates and communicates with the VBA and community providers to obtain records and information to determine a Veteran's eligibility, works with these agencies to update character of discharge for eligibility purposes.
- Processes applications for VHA services
- Requests DD-214's and other documents necessary to determine eligibility and complete the enrollment process
- Verifies eligibility internally for Homeless Program staff, as well as community homeless service partners
- Registers/enrolls Veterans in the VHA database and the Homeless Program
- Performs other duties, as assigned

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
