Health Care for Homeless Veterans (HCHV) Program Community Engagement/Coordinated Entry Coordinator (CEC)
The HCHV Community Engagement/Coordinated Entry Program Coordinator enables the VA Eastern Kansas Health Care System Homeless Programs to fully collaborate with community partners, including all Continuum of Cares (CoC), so VA EKHCS can effectively implement and monitor compliance with community coordinated entry guidelines.
The VA Eastern Kansas Health Care System VAMC Community Engagement Coordinated Entry Coordinator (CEC) is responsible for:
Community Engagement - Participating in VA EK Catchment area of service (County COCs) and the Balance of State Coe meetings and planning efforts which may include leadership roles within the CoC groups. Participating in case conferencing process within the CoC's coordinated entry system to help inform and facilitate referrals to VA programs such as HUD-VASH, SSVF, CERS, and GPO. The incumbent will be the main point of contact for all CoC By Name List (BNL) meetings and should be prepared to discuss individual Veteran case status.
Coordinated Entry - Ensuring timely and accurate entry by all members of the HCHV Outreach Team into VA data systems, such as HOMES, and reviewing the data to improve performance and delivery of services to homeless Veterans. Ensuring that this information is shared (in a manner consistent with VA information sharing directives) so that community By-Name Lists are up-to-date and complete.
Direct Patient Care Providing coverage for the HCHV Outreach clinic, when needed, by completing psychosocial histories and assessments to aid in the development of treatment plans. Coverage may also include provision of case management services to include: assisting Veterans in searching for housing, gathering documents, acquiring food resources, going to medical appointments, or transporting to emergency shelter options. The CEC will at times need to transport Veterans within the VAEKHCS catchment area in government vehicles. The Community Engagement Coordinated Entry Coordinator reports directly to the Social Work Supervisor but practices independently, exercising initiative and independent judgment in day-to day activities. Supervision/consultation is of an administrative and professional nature with the Social Work Supervisor providing guidelines as to the general policy and direction of the Coordinated Entry Program. In unusual and/or complex administrative and clinical problems, the incumbent initiates consultation with Chief of Social Work Service as well as with other authoritative resources at the Medical Center, such as Behavioral Health, Homeless Programs, Patient Services, Business Services, Fiscal Services, and Regional Counsel.
QUALIFICATION REQUIREMENTS: Meets the qualification standard for the GS 12 Social Work Program Coordinator as defined in VA Handbook 5005/120, Part II Appendix G39, Social Worker Qualification Standard GS-185 VHA.
The Community Engagement/Coordinated Entry Coordinator must hold a valid unrestricted driver's license. The CEC will need to pass a pre-employment driving physical through Occupational/Employee Health to transport Veterans in a Government vehicle and must have a Basic Life Support (BLS) Certification.
