HBPC is a home care program under the Geriatric and Extended Care Service Line, designed to meet the longitudinal, primary care needs of an aging Veteran population with complex, chronic, disabling disease. HBPC provides a patient centered approach to care and treatment in the patient's home. The HBPC RN is a foundational member of the interdisciplinary care team for reasonably home bound, chronically ill Veterans. The HBPC RN functions as a care manager for the HBPC assigned Veterans and is responsible for continuous assessment of the Veteran and caregiver needs; delivering nursing care in the home; and supervising the care of home health technicians if applicable.
