The Licensed Practical Nurse (LPN) working in the Community Based Outpatient Clinics (CBOC) is responsible for providing safe, effective clinical and administrative support in a wide variety of patient care and related activities. The LPN functions as a member of the health care team by demonstrating the practical nursing knowledge and skills necessary in carrying out complex practical nursing care assignments within the CBOC. The purpose of this position is to be a productive team member carrying out and ensuring quality and timely delivery of Veteran centric care. The incumbent must possess the personal qualities, desire and attitude necessary to adapt to rapidly changing demands on time and workload and have necessary skills to perform specific tasks/tests/procedures to meet the Veteran's complex, healthcare needs.
Under the direction of the Registered Professional nurse, the LPN provides a broad range of direct, age specific patient care activities and supportive services, as identified in age-specific competencies, including demonstrating knowledge of changes associated with aging and principles of growth and development relevant to the adult and geriatric age groups. Using traditional face to face and/or telehealth modalities, demonstrates the ability to interpret, report and document data about the patient's status; the ability to identify age-specific needs and provide appropriate care based upon the age-related factors noted including assisting with procedures, providing patient education and other duties as needed. The population of male and female Veterans served ranges from the adult, age 18, to the elderly adult, age 65 and older. Age-related and cultural interventions appropriate to the cognitive, physical, and emotional needs will be employed at all times, tailored to the individual patient, providing patient centered care while incorporating knowledge of changes associated with aging and principles of growth and development.
The LPN functions under the guidance and supervision of the Registered Professional Nurse and is accountable for demonstrating the knowledge and skills necessary to perform patient care and supportive responsibilities appropriate to the age of the patients served. The LPN is responsible to the Nurse Manager for overall work performance and evaluation maintaining acceptable standards within a health care setting.
