Incumbent will Independently conduct psychosocial assessments and make psychosocial diagnoses based on assessment. Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team. Incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions. Incumbent is able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
Independently identifies high-risk patients and provides therapeutic services. Incumbent provides crisis intervention services and Evidence Based Treatment modalities to address treatment and crisis. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures.
Establishes and maintains effective therapeutic relationships with Veterans and their families. Incumbent can independently work with patients who are experiencing a variety of psychiatric, medical, and social problems utilizing primarily group evidence based treatment modalities.
Independently provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Ability to evaluate incumbent's own practice through professional peer review, case conferences and other organizational means. Incumbent can communicate effectively, both orally and in writing, with people from varied backgrounds.
Follows STVHCS standard operating procedures for evaluating and mitigating suicide risk to include: completes the Columbia Suicide Severity Risk Scale (C-SSRS), the Comprehensive Suicide Risk Evaluation, and a Safety Plan via shared template in the Computerized Patient Record System (CPRS) on all patients at intake, with Veterans with suicidal ideation and on any Veteran whose record is flagged "High Risk for Suicide." Additionally, completes the Suicide Behavior and Overdose Report (SBOR) as clinically indicated.
Incumbent is responsible for making decisions regarding work problems presented by subordinate social workers. Incumbent collaborates with others in negotiating, deciding upon, and/or coordinating social work-related changes affecting other programs. The Social Work Supervisor advises Care Line Managers/Chiefs on problems involving the relationship of social work activities to the various clinics and programs.
Assists in selections for non-supervisory positions, hears group grievances and serious employee complaints, review serious disciplinary cases (i.e., those proposing suspensions and removals), and disciplinary problems involving key employees. The individual consults with specialists regarding training needs and decide types of training problems related to the identified problems of the supervised wards/units.
