This position functions as the Pharmacy Program Manager for the VISN 22 Clinical Resource Hub (CRH). The VISN 22 CRH Pharmacy Program Manager will be based out of and credentialed at the Phoenix VA Health Care System (PVAHCS), but will be supporting the VISN 22 CRH and all stations in VISN 22.

The incumbent is responsible for developing, organizing, coordinating, and managing all aspects of clinical pharmacy functions related to the VISN 22 Clinical Resource Hub (CRH). The Pharmacy Program Manager is responsible for policies, functions, patient safety, and quality as it relates to the program. The Program Manager will also coordinate with other branches of the Nationally Designated Clinical Resource Hub (ND-CRH), including the Tele Mental Health Hub (TMH), Tele-Specialty (TS), and the Clinical Contact Center (CCC), to optimize VISN 22 CRH Pharmacy services.
As the VISN 22 CRH Pharmacy Program Manager, the incumbent is responsible for Clinical Pharmacy Practitioner (CPP) services including the program's leadership, management, training, and operations. The incumbent will be required to exercise a high level of leadership, organization skill, independent judgment, problem-solving ability, and content expertise in program development, implementation, and evaluation. The VISN 22 CRH Pharmacy Program Manager will work directly with various levels of leadership including CRH leadership, VISN leadership (e.g., VISN Deputy Chief Medical Officer, VISN Pharmacy Executive, etc.), facility leadership (e.g., Chief of Pharmacy, etc.), and the National CRH Leadership Team to ensure CPP services meet expectations and proper requirements. This individual has a supervisory role for the V22 CRH CPP.
The CRH Pharmacy Program Manager also serves as a CPP provider for the CRH program. The VISN Program Manager functions at the highest level of clinical practice and works independently under a scope of practice (SOP) to directly care for patients. As a health care team member, the VISN Program Manager performs comprehensive medication management (CMM), including the initiation, modification, and discontinuation of medication therapy, through interaction with both patients and fellow health care team members. Incumbent serves as a provider with prescribing privileges as outlined in the individual's SOP for the provision of evidence-based pharmaceutical care services through interaction with both patients and providers. Incumbent is often in association with or in support of pharmacists, behavioral health, primary care providers, nurses, and other health care practitioners.
