Primary Care - Mental Health Integration (PCMHI) Care Management is an emerging discipline which works at the interface between primary care and mental health care in a blended, integrated care model. PC-MHI Care management assures adequate identification, monitoring, and treatment of a variety of emotional and psychiatric problems. The Care Manager provides assessment and monitoring of patient adherence, side effects, and clinical effectiveness of new psychiatric medication starts. In addition, the PCMHI care manager provides education, brief interventions and support to patients. The PCMHI Social Work Care Manager also provides "on-time, on-target' information to Primary Care Providers {PCPs) regarding their patient's response to treatment, and collaboratively makes appropriate care decisions with the PCP and their PACT team.. Additional duties involve coordination of care with integrated behavioral health co-located staff and the rest of the PACT team. He/she functions as a treatment advocate and guide, consultant, counselor and educator while providing integrated health care to outpatients in Primary Care. The assessment and collaborative care provided by this professional social worker is performed primarily via VA Video Connect, In-person appointments, and telephone contacts, but may also include the use of other clinical technologies
The GS-11 Social Worker at full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level {LCSW, LISW, etc). A GS-11 social worker has a Masters Degree in Social Work granted by a graduate program fully accredited by the Council on Social Work Education {CSWE). Other qualifications include: A minimum of three years of successful clinical practice; demonstrated ability in the areas of interpersonal relations, critical thinking, and problem-solving and conflict resolution; knowledge of current treatment for mental illness and the key symptoms of common psychiatric illness such as depression, GAD, PTSD, and Panic D/0; knowledge of psychosocial assessment and clinical management of patients; knowledge of medications prescribed for patients with depression, including patient responses and possible adverse reactions; ability to use VA computer system, CPRS, BHL Software, Excel and VistA-based clinical informatics.
Work Schedule: Monday - Friday 8:00 a.m. to 4:30 p.m. Typically, this position is approved to tele-work up to 4 days/week and work on site at least 1 day/week.
