The Restorative Care Coordinator demonstrates strong clinical leadership and rehabilitative management skills. The Restorative Care Coordinator ensures that an effective restorative nursing care program is offered to the residents of the Community Living Center and is in compliance with the Centers for Medicare and Medicaid (CMS) Standards. The coordinator collaborates with nurse leaders, rehabilitation therapists, restorative health technicians, and other members of the interdisciplinary team to deliver restorative care programs designed to meet the needs of Veteran residents.
The Restorative Care Coordinator demonstrates a high level of clinical competence in outlining measurable, achievable, and functional goals for each Veteran resident referred to the Restorative Nursing Program. Responsibilities include completing assessments and reassessments with a focus on maintaining or improving resident's functional status by utilizing medical chart review, face-to-face contact and/or telephone conferencing with Veterans, families, interdisciplinary team members, rehabilitation therapists and restorative health technicians.
The Restorative Coordinator is accountable for utilizing the nursing process to coordinate and establish effective individualized restorative plans of care for Veteran residents who qualify for the Restorative Care Program.
