The Mental Health Comprehensive Medication Management (CMM) Clinical Pharmacy Specialist (CPS) position provides and promotes rational drug therapy to patients at Hines VA Hospital and affiliated institutions including Community Based Outpatient Clinic (CBOC) through the provision of innovative and progressive patient care services that foster and support high quality pharmaceutical care and pharmacotherapy services. This individual must have a clear understanding of processes, procedures, and standards of clinical practice and the VHA. The individual must understand current practice standards from ASHP, Joint Commission, VA PBM Guidance, VHA Directives, VHA Handbooks, Hospital Policy and Procedures, EPRP, Care Coordination Agreement (CCA), Quality Measures and/or other professional standards and advise the organization and the department of their impact on patient care.
The Mental Health CMM CPS is highly trained Advanced Practice Provider who will work collaboratively under a global Scope of Practice (SOP) with prescriptive authority as defined by the medical center. The CPS has expertise in CMM which encompasses the complete medication management of patients with chronic disease states including Mental Health. The CPS will plan, develop, and maintain effective clinical pharmacy programs. Mental Health CPS initiate, modify, and discontinue medications and perform CMM services via face-to-face visits, telehealth, VVC/VMR, Shared Medical Appointments (SMAs), and electronic consultations for direct and indirect patient care activities. The CPS improves clinical outcomes through the provision of CMM and applies principles of team-based care and population management to improve access, quality, and safety.
The incumbent will professionally practice under a Scope of Practice within all of areas of the Mental Health Service Line. Primary practice to include Hines VA campus Outpatient Mental Health Clinics and virtual coverage to Mental Health patients in the CBOCs with occasional physical presence. The incumbent will be expected to collaborate with the patient's primary care physician on any identified non-mental health medication management issues.
The CPS will demonstrate formulary management / pharmacoeconomics program development that results in appropriate, safe, cost-effective medication approvals / conversions / therapeutic interchanges.
The CPS will be accountable for pharmaceutical care decision-making and have hard data to demonstrate the outcome from implemented program on cost-containment and/or patient outcomes (the quality of care).
The CPS will work with other health care professionals as an essential component of a multidisciplinary team. The incumbent will be expected to precept residents and students, as well as provide education to providers, pharmacists, patients, and other key stakeholders.
Work Schedule: Full-Time, Monday-Friday may include evening, weekends, holidays.
