The Northport Veteran's Affairs Medical Center (VAMC) is a Level-1c facility affiliated with the School of Medicine at the State University of New York at Stony Brook. We have and exciting opportunity for an experienced Registered Nurse in our Specialty Clinics! The RN uses the nursing process and evidence-based practice to collaborate with the Multidisciplinary team in developing the patient-centered holistic care plan for life.
A few Registered Nurse vacancies exist in our Northport VA Medical Center Specialty Care Clinics. The RN's uses the nursing process and evidence-based practice to collaborate with the Multidisciplinary team in developing the patient-centered holistic care plan for life. They are responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the Veterans' needs. Guided by ICARE values, the RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the Veteran's goals. The RN demonstrates leadership by serving as an advocate for Veterans and as a team player to colleagues as she/he continues to enhance his/her own professional growth, development and practice.
Work Schedule: Days/Evenings/Nights (required tour of duty will be discussed during the interview process)
