The VIRS (Veterans Integrated Referral System) RN Coordinator serves as a clinical liaison between VA and non-VA healthcare providers managing Veterans healthcare if hospitalization is necessary. This position's primary function is to aid in transferring care of our Veterans from a community facility to a VA facility where indicated and possible. Incumbent will function in an independent manner under the direction of the Office of Community Care, Administrative Medicine Section Nurse Manager.

The duties of the VIRS Rn are:
1. Receives alerts to all hospitalizations for patients, acting as the hub for documentation into the Veteran's electronic medical record for purposes of continuation of care by Veteran's primary care team and payment of community care.
2. Perform clinical review of medical records of patients who are hospitalized in non-VA hospitals to determine appropriateness of care for payment, point of stability for transfer of the Veteran to a VA facility if continue acute care or rehab is required.
3. Make recommendations for payment based on a clinical review performed within the context of existing VA regulations regarding non-VA medical care when such care has not been prior-authorized.
4. Act as a liaison and resource between VA and non-VA facilities to coordinate inpatient care that is proposed or has already been scheduled.
5. Coordinate complex care in non-VA facilities when clinical interface or consultation with the VA is required.
6. Responsible for the triage and care coordination of complex patients, works closely with the Veteran to ensure smooth transition in transfers and care related referrals, attends VIRS conference calls, improvement meetings, and service meetings.
7. Case management of ongoing complex medical needs of the patient.
8. Execute position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex patient care for patients receiving care through designated VA tertiary sites.
9. Provide care in the least restrictive, most cost-effective manner using clinical practice guidelines and clinical pathways.
10. Assist with significant collaboration with community and VISN healthcare personnel in the transition of Veterans through the VA Healthcare system.
11. Process inter-facility consults ensuring all necessary pre-consult testing, labs and imaging associated with the referral are communicated to the specialty clinic.
12 Provide equality and performance measures analysis and reporting of transfer trends and systems issues through the utilization of the nursing process.
13. Coordinates the care of veterans in community emergency rooms and inpatient settings.
14. Actively advises and refers to programs available inside and outside of the VHA system.
15. May be called on to administer the duties of the Traveling Veteran Coordination (TVC) role, working with the Veteran and coordinating medical care for veterans traveling to and from the Walla Walla VA system with the goals of providing seamless care, and attending monthly TVC conference calls.
16. Educates veterans and community staff members on the rights and responsibilities of the medical services available based on eligibility rules and regulations and offers options for services provided through the VA including appropriate transportation, level of care, and appropriate services with respect to resource utilization and safety.
17. Assists in bridging the gap for connecting patients to needed services, such as, respiratory, prosthetics, primary care and other specialty services.
18. Attains guidance from the Compensation and Pension provider to help interpret service-connection ratings for care.
