The Sleep Medicine Case Manager/Care Coordinator (SMCM/CC) is a registered nurse responsible of using all elements of the nursing process (assessment, diagnosis, planning, implementation, and evaluation), revision of policies, procedures and guidelines to ensure the delivery of safe, efficient and effective evidence-based care. Influences care outcomes by collaborating with members of the interdisciplinary team, executing position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
Supports and provides guidance to patients within the Sleep Medicine Service, with Sleep Medicine related disorders, with the primary goal of facilitating timely access to quality care, and a well informed and effective patient flow throughout the spectrum of care.
Demonstrates case management and compliance functions by serving as a liaison and patient advocate navigating patient complaints and specific needs. Improves interdisciplinary team knowledge and understanding by providing required medical care for patients seeking care in a more timely manner, triaging, consult management and patient education, enhancing access and addressing interventions to reduce time to delivery of early diagnostic and therapeutic services through community and institutional screening programs for the Sleep Medicine care continuum.
Responsible for the implementation of an ongoing review program, assuring access to care to Veterans by combining elements of coordination from the provision of health care services with record and care review, ensuring the oversight of vulnerable populations during emergencies are contacted and accounted for. Other duties include but are not limited to: assisting Veterans, family members, and caregivers by offering the most appropriate options and services to meet their complex health care needs such as combating stress or residuals of traumatic brain injury, acute, catastrophic or life-threatening illnesses, addiction and other health problems. Coordinates with multiple providers across all levels and sites of care, as well as evaluating care and outcomes to ensure timely and appropriate provision of services. Recognizes and collects relevant information for durable medical equipment (DME) initiation and follow-up, with the sufficient knowledge and skills to educate patient/family on services such as positive airway pressure (PAP) therapy/machines and other equipment.
The Sleep Medicine Case Manager/Care Coordinator (SMCM/CC) is accountable to the Assistant Chief Sleep, Neurology and Sleep Specialty Care Chief, ACOS for Medicine and Subspecialty Care and ultimately, to the Associate Patient Care Service responsible for the professional practice of all nurses within the VACHS.
