This position is located within the Claremore Service Unit, under the supervision of the Outpatient Nurse Manager or designee. As the RN-Care Manager (RN-CM), the incumbent is responsible for all aspects of Care Management for empaneled patients assigned to their designated care team, assisting in the development of the Plan of Care, ensuring that the plan is implemented in a timely basis, and identifying patient needs.

A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. LicensureFor all grade levels and positions, applicants must have passed the National Council Licensure Examination(external link).
