Duties include, but are not limited to:
Customer Service
1) Meets the needs of customers while supporting VA missions. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Responds to the needs of the service in a courteous and timely manner. Follows up, as necessary, to ensure a satisfactory resolution.
2) Demonstrates appropriate and proactive interpersonal skills to achieve the goals of the service. Acts as a team member and is supportive of the group's efforts at all times.
3) Demonstrates an ability to work under a variety of circumstances to maximize group productivity and quality.
4) Provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with customers constructively and appropriately.
5) Relationships with upper management, supervisors, co-workers, and others within the organization are courteous and cooperative in nature and contribute to the effective operations of Pharmacy Service and the medical center.
6) This position works within and supports a workgroup (workgroup is considered Pharmacy staff assigned to complete similar work).
Clinical
1) Performs the following list of tasks as required:
a) All tasks having to do with medication preparation and dispensing
b) Medication counseling
c) Completing medication histories including patient interviews
d) Documenting activities in CPRS
e) Extending refills until next scheduled appointment under the provider's name. If no appointment exists when issuing the refill extension, the pharmacist must have a process to alert clinic staff or arrange for a future appointment when needed.
f) Flagging prescriptions
g) Managing view alerts
h) Using the "hold medication" function
i) Generating unsigned discontinued orders using Service Reject
j) Monitoring drug therapy
k) Assessing drug safety and efficacy including evaluation of physical symptoms
l) Providing drug information and education to patients and staff
m) Reporting adverse drug events, near misses and medication errors
n) Providing oral and written consultations with other healthcare professionals regarding drug therapy selection and management
o) Maintaining BLS certification and participating in medical emergencies including code teams (if ACLS certified) and emergency preparedness activities
p) Entering per policy orders involving supply items under provider name.
q) Providing pharmacokinetic consultation including dosing vancomycin and aminoglycosides under provider name in accordance with local policy
r) Implementing P&T Committee policies including therapeutic interchanges and automatic substitutions
s) Managing recalls and medication shortage situations by substituting alternate dosage strengths and instructions of the same medication to equal the prescribed dose and schedule
t) Participating in research in accordance with FDA guidelines and regulations and VHA policy
2) Is directly involved in the design, implementation, and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
3) Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate.
4) Updates the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the patient is not taking.
5) Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as need.
6) Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures.
7) Identifies drug related problems such as allergies, toxicities, improper dosages regimens, etc, which may require individualized treatments, adjustment of dose or change of drug, and communicates recommendations to the provider.
8) Is able to apply a knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.
9) Has expertise in the principles of clinical pharmacokinetics and recommendations based on objective laboratory findings.
Work Schedule: Full-time; 40 hours/week. In rotation for weekends, on-call, holidays and late shifts.
