The Patient Aligned Care Team (PACT) - based model of care features three (3) major principles: patient-centered care, coordination of care and access to care. The Social Worker assigned to this position will act as an integral part of these PACT in the clinical and treatment care process. The incumbent is responsible for Social Work needs for PACT in Primary Care, which may include providing specialty care clinics at to the Robert J. Dole VA Medical Center. The incumbent provides a full range of services to patients and their families, who present with problems that range from basis psychosocial needs to more complex biopsychosocial needs.
