The Ambulatory Care (CBOC) Clinical Coordinator is responsible for performing key duties in the areas of administration, supervision, management analysis, and day to day operational management of the ambulatory care site. This includes programmatic evaluation; evaluation of clinic work flow and the continuum of care; management of resources such as staff, space, and equipment; oversight of nursing care; quality assurance, quality improvement activities, and performance measures; and ensuring compliance with Joint Commission and other regulatory agencies. The incumbent works independently and collaboratively with the Ambulatory Care SL Nurse Administrator, Ambulatory Care Director, and the Administrative Officer (AO) for Ambulatory Care in improving holistic care through collaborative strategies. During the absence of the AO, he/she is responsible for ensuring a safe, well-maintained working environment, identifying needed maintenance, and following through to correction, working with Contracting Officer's Representative, lessor, and contractors. The incumbent is responsible for planning, organizing, initiating, and directing administrative and clinical program functions which impact the patient care process within the site, integrating VHA and service line goals and objectives into the operation of the site. The Clinical Coordinator serves as an organizational steward; integral to which is the promotion of interdisciplinary outcome-based practice, customer service and satisfaction, and quality of care. The Clinical Coordinator provides education and training, to include new employee orientation. He/she demonstrates knowledge of the PACT (Patient Aligned Care Team) practice model and actively works toward achieving PACT goals. Works with other team members to educate patients on disease prevention and wellness promotion facilitation of teaching Health Living Classes (Shared Medical Appointments), individual disease awareness classes or referral for other appropriate VA classes, and utilization of Population Health principles. The Clinical Coordinator has the responsibility of building and maintaining positive public relations and contacts with various individuals at all levels of authority. He/she collaborates with other executives in the organization in making decisions about health care services, settings, and organizational priorities in order to meet the needs of the consumers. The Clinical Coordinator actively participates in programmatic development, evaluation, and implementation of policies and procedures in current and new programs. It is the responsibility of the Clinical Coordinator to create a work environment that facilitates and encourages critical thinking and interdisciplinary collaboration. He/she is direct supervisor to nursing and other allied health staff assigned to the site. He/she is visible and available to front line staff on a day-to-day basis. The Clinic Coordinator will be the main back-up for the RN Case Manager, LPN, and lab tech positions at this location.
