The SW Coordinator provides social work services to the veteran population seen in one of the Community-Based Outpatient Clinics of the San Francisco VA Health Care System catchment area which includes Mendocino County and Lake County. The incumbent is responsible for outreach into the community to find and bring veterans into the clinic. Services consist of high quality assessment and intervention: assessment of coping in the community, including assessment of suspected abuse, especially elder abuse and issues related to capacity; interventions such as counseling to patients and families around adjustment to disability, illness and loss; crisis intervention; advance directive education; coordination of linkages to other appropriate VA or community service providers/agencies; clinic-based, short-term, focused case management; and consultation to multidisciplinary team members on the psychosocial aspects of individualized patient care. Continuity of care is key, to assure patient safety, completion of referrals, and firm hand-offs to resource agency programs and providers.
Work Schedule: Monday through Friday, 8:00am to 4:30pm

VA may offer newly-appointed Federal employee's credit for their job-related non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which they earn annual leave. Such credit must be requested and approved prior to the appointment date and is not guaranteed.
