The incumbent supervises CLC, ADHC and SA-PACT social work staff that provide and coordinate institutional, programmatic and/or community resources for their respective resident/patient populations. The supervised staff treat a variety of clinically challenging patients who have serious and persistent medical, functional, psychosocial and/or mental health conditions and deficits. The incumbent coordinates the day-to-day social work operations, administration, staffing, program mission, and fiscal management of the CLC, ADHC and SA-PACT.
The GEC Program Manager works closely with the staff from acute medical, surgical, and psychiatric inpatient services, the Brooklyn (BK) and New York (NY) campus Emergency Departments, the BK/NY PACTs, Specialty Care Clinics, the Non-Institutional Care programs, and other VA Medical Centers (VAMCs). Interactions also include partnering and collaborating with referring community providers from community hospitals, clinics, and others such as skilled home care and hospice partners, Veterans Services Organizations (VSOs), New York City (NYC) and New York State (NYS) agencies, Veterans, and their families. The GEC Program Manager works closely with the SA Leadership Team, NY Harbor Clinical Leadership as requested, and VISN 2 Network Rehabilitation & Extended Care (REC) Integrated Care Council (ICC) Executive Council.
As designated by the Chief of Social Work Service and related to areas of responsibility, the incumbent serves on or attends various VA New York Harbor Healthcare System (NYHHS) groups or committees, such as the SA-Quality Leadership Team (QLT) and VISN 2 CLC Quality Assurance and Performance Improvement (QAPI) Workgroup, and any assigned local, VISN or national program advisory councils.
As designated by the Chief of Social Work Service, the incumbent may represent VA NYHHS and the Service on Veterans Integrated Service Network (VISN) task groups or committees, such as the VISN 2 REC ICC Executive Council. As designated by the Chief, the incumbent may also represent VA NYHHS and the Service on various national GEC meetings such as the national CLC and Hospice and Palliative Care monthly meetings.
The incumbent must be licensed in a state at the advanced practice level and must be able to provide supervision for social work licensure.
Duties include but are not limited to:
Supervisory:
Supervises a group of employees performing work at various levels, and provides clinical, administrative, and technical supervision necessary for accomplishing the work of the programs. The incumbent exercises supervisory authorities and responsibilities involving work assignment and review, as well as the administrative and personnel management functions relative to the staff supervised.
The incumbent plans work to be accomplished by employees under the span of control, setting and shifting priorities and schedules to meet the mission requirements of VA NYHHCS as they apply to the management of social work staff in the CLC, ADHC and SA-PACT. The incumbent establishes guidelines and performance expectations for staff, observes workers' performance, and demonstrates and conducts work performance critiques.
Program Coordination & Planning:
Responsible to coordinate work responsibilities of social work clinical and support staff assigned to the programs; ensuring employees understand and adhere to the program regulations, directives, policies, and standards.
Community Agency Coordination & Support:
Maintains open lines of communication and effective interaction with community healthcare facilities and city/state agencies that are currently providing care to Veterans and/or inquiring about eligibility criteria and/or VA services in general. Acts as a primary point of contact and liaison for internal and external customers/stakeholders, addressing problems and challenges related to enrollments and referrals.
