The Senior Social Worker at the Fremont Community Based Outpatient Clinic (CBOC) is responsible for developing and directing medical social work services at the outpatient clinic serving Veterans and their families from a large geographic area. The incumbent works collaboratively with medicine, mental health and specialty clinics in planning, organizing, managing and evaluating the social work services needed to further the mission of the clinic in providing exceptional health care to improve the health and well-being of the veterans served by the clinic. These social work services include: screening, assessing, consulting, managing crises, triaging, treating, referring, coordinating VA services and community resources, and developing policies and procedures to improve the social work services to the clinic providers, veterans and their families.
Major duties include, but are not limited to;
Functions as a primary contact for community agencies, hospitals, health care facilities, city and county governments, providing general information about VA benefits & eligibility and the VA Palo Alto Health Care System. The incumbent is one of the identified points of contact for discharge planning from community hospitals communicating and coordinating care with the Primary Care team, Mental Health team and Veteran/Family. The incumbent coordinates and provides information in compliance with HIPAA and VA Privacy regulations regarding VA benefits pertaining to nursing home care, in-home assistance, and Aide and Attendance/housebound benefits. The incumbent evaluates Veterans' eligibility for VA nursing home care and identifies the medical needs to distinguish between extended care services, other VA inpatient programs, Medi-Cal long term care options, in-home health services and county services. Incumbent facilitates communication between outside hospitals and VA Fremont CBOC primary care providers.
Functions as a clinical supervisor to IPACT Peer Support Specialists. May also develop standard operating procedures, policy and procedures to develop the I-PACT Peer Specialist Program in conjunction with the I-PACT Peer Specialist Social Work Administrative Supervisor. This includes serving as one of the lead facility points of contact to oversee and support the Peer Specialist LIP clinical supervisor at each CBOC (Fremont, Palo Alto and San Jose CBOC), providing clinical supervision to Peer Support Specialists, and supporting the two IPACT Peer Support Specialists and PACT Teamlets on a daily basis, tracking consults, overseeing program development, developing VAPAHCS best practices, standard work and improving upon quality of care, safety and to support increasing access to improve Veteran centered care.
Develops, adapts and directs social work services to meet the changing needs of the Integrated Primary Care and Mental Health Clinics and any specialty clinics at VA Fremont CBOC while engaging the i-PACT Teamlets to work with patient and family to address bio-psycho-social needs.
Serves as a leader for I-PACT Medical Social Worker for the Fremont Clinic for advanced knowledge of VA and Community resources, consultation regarding difficult cases when the supervisor is not available and offering regular competency trainings to I-PACT SW Section and Staff.
Tracking and managing I-PACT Fremont and off-site medical social work consults and troubleshooting data management.
Coordinates medical social work services including referrals to community resources and provides support services in these areas to the mental health clinic.
Coordinates long term care referrals to VA Community Living Centers (CLC) at Menlo Park and/or Livermore and facilitates long term care referrals to contracted nursing homes with the Community Nursing Home Coordinator.
Work Schedule: Monday through Friday 8:00 AM to 4:30 PM
