The incumbent in this position serves as the Advance Care Planning via Group Visit (ACP-GV) Program Coordinator and serves as the clinical and administrative supervisor of staff including, but not limited to, the Intimate Partner Violence Program (IPVAP) Coordinator, Dialysis Social Worker, Oncology Social Worker, and the Health and Behavior Coordinator.
