Area of consideration is for current permanent Department of Veteran Affairs Employees. First consideration will be given to Birmingham VA Healthcare System Employees and CBOC employees, Canteen Service (VCS) employees who have completed 90 days of continuous service in the VCS, Healthcare Professional Trainees (HPT), and all permanent IT employees who are assigned to the Office of Information and Technology(OI&T).
Duties may include but are not limited:
Incumbent will support the Contract Nursing Home by providing discharge planning and routine visits as assigned.
The incumbent will be responsible for working closely with Business Management Service to provide oversight for agencies that are under Provider Agreements for Homemaker/Home Health. He/She will be responsible for ensuring treatment plans reflect services ordered; and provide oversight for Veterans that receive services under HHA and Respite Care.
The Non-VA Care Coordinator will chair the Homemaker/Home Health Aid Committee and report findings to the Community Non-VA Care/Contract Nursing Home Programs Oversight Committee.
The incumbent will provide clinical social work and case management to Veterans discharged from the BVAHCS to Non-Contracted skilled nursing facilities, including VA programs. He/She will ensure Veterans receive proper follow up and education about Medication Reconciliation and screen for services post discharge from the non-contracted skilled nursing facility.
Will follow-up with nursing homes, receive medication list, and provide to Primary Care (PC) team for medication reconciliation.
The incumbent will provide extensive case management to Veterans on the Acute Care List (ACL). He/she will work closely with the Business Office to coordinate discharge plans.
Incumbent will work closely with outside hospitals for effective discharge planning for Veterans on the VA Acute Care List.
The incumbent will support the Community Care team by providing discharge planning and routine visits as assigned.
Work Schedule: Monday - Friday; 8:00 am to 4:30 pm
