The primary purpose of the Licensed Vocational Nurse (LVN) is to perform assigned duties in a manner that promotes quality health care, position customer service and the maintenance of an environment committed to serving the veteran. The Community Living Center is a community of people -- residents, their families and friends, and an interdisciplinary staff of caregivers. They interact to create an environment that supports comprehensive and responsive care and that recognizes and supports the psychosocial needs and interests of all the residents. Provide the highest quality health care, including rehabilitative, restorative, and terminal care that is responsive to the needs of a primarily geriatric population.
Behavioral Unit neighborhood provides comprehensive care to residents with chronic aggressive behavior, and mental health conditions. The team uses a collaborative behavioral and HATCH model approach to providing nursing home care in developing a comprehensive and individualized treatment plan that includes behavioral management protocols, promotes resident- centered care that supports quality of life and quality of care consistent with long term care standards with a focus on preserving veteran's rights, dignity and self-image.
The Community Living Center is characterized by a sustained and consistent effort to provide exceptional care and demonstrate an outstanding standard of practice, team building, and collegiality under the supervision of the registered nurse. The staff LVN must demonstrate through behavior their commitment to Central Texas Veterans Health Care System VA Core Values of Integrity, Commitment, Advocacy, Respect and Excellence.
The position requires an individual who is motivated and an excellent team player, with flexibility to assume other functions related to patient care on an as-needed basis within the Community Living Center environment. Possesses excellent communication skills and is able to effectively interact with Veteran's and their families. Demonstrates time management and organizational skills in the work day.
DUTIES: The LVN performs patient care duties under the direct supervision of the RN. Must have astute prioritization skills, critical He/she provides patient care which is appropriate to the cognitive, emotional, spiritual, chronological, recreational, and diversion needs of the adult (ages 17-59) or geriatric (ages 60+) patient. The LVN demonstrates knowledge of changes associated with aging and provides care based upon age related factors as specified in his/her competencies. Contributes to the interdisciplinary team in developing, implementing, and evaluating the patient's treatment plan. Promotes patient participation in the therapeutic community by fostering self-reliance and independence. Performs assigned duties, treatments, or procedures in acceptable timeframes, following established policies and practices for Primary Care. Performs more complex treatments as assigned.
Work Schedule: Rotating Tours of Duty to include days, weekends, evening and night shifts. Primary shifts schedules are 7:30 am - 4:00 pm, 10:00 am - 6:30 pm, 3:30 pm - 12:00 pm, and 11:30 pm - 8:00 am. Other Tours of Duty that may be utilized are 5:00 am - 1:30 pm, 6:00 am - 2:30 pm, 9:00 am - 5:30 pm, and 1:00 pm - 9:30 pm.
