Home Based Primary Care (HBPC) provides home care and community services for Veterans who have a number of chronic medical conditions or require post-surgical care. Patient populations include male and female, adult and elderly, frail patients. HBPC provides home health care services but does not provide infusion therapy, home health aide, chore, or hospice care services. The RN staff provides medication management, injections, wound care, anticoagulation and glucose monitoring, blood draws, specimen collection, safety evaluations and assessments, and patient education. The RN is accountable for a primary caseload, for patient assessments, patient/family education, and coordination of the therapeutic regimen in the home setting.
Work Schedule: Monday - Friday; 0730-1600 or 0800-1630
